THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
How We Use and Disclose Your Protected Health Information (PHI)
Houston Physical Therapy & Performance Center, LLC may use and disclose your PHI for the following purposes:
Treatment: We may use your PHI to provide, coordinate, and manage your physical therapy and occupational therapy treatment. This includes sharing information with other healthcare providers involved in your care, such as your physician, surgeon, or specialist.
Payment: We may use and disclose your PHI to bill and collect payment for the treatment and services we provide. This may include sharing information with your health insurance company, Medicare, Medicaid, or other third-party payers.
Healthcare Operations: We may use and disclose your PHI for our healthcare operations, including quality assessment, staff training, licensing, accreditation, and business planning activities.
Your Rights Regarding Your PHI
Right to Access: You have the right to inspect and obtain a copy of your PHI maintained by our practice. Requests must be made in writing.
Right to Amendment: You have the right to request that we amend your PHI if you believe it is incorrect or incomplete.
Right to Restriction: You have the right to request restrictions on certain uses and disclosures of your PHI. We are not required to agree to all restriction requests, but we will consider each request carefully.
Right to Confidential Communications: You have the right to request that we communicate with you about health matters in a particular way or at a certain location.
Right to Accounting of Disclosures: You have the right to request a list of certain disclosures we have made of your PHI.
Right to File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with our Privacy Official or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized for filing a complaint.
Our Duties
We are required by law to maintain the privacy of your PHI, provide you with this Notice of our legal duties and privacy practices with respect to your PHI, notify you in the event of a breach of your unsecured PHI, and abide by the terms of this Notice currently in effect. We reserve the right to change our privacy practices and the terms of this Notice at any time. Any changes will apply to all PHI we maintain.
Other Uses and Disclosures
Other uses and disclosures of your PHI not covered by this Notice or applicable laws will be made only with your written authorization. You may revoke an authorization at any time in writing, except to the extent that we have already acted in reliance on the authorization.
